A speciality of general practice is managing multi-morbidity (more than one chronic health condition). This affects 65% of over 65s and continues to increase with age. With increasing chronic conditions, health care is increasingly fragmented, with input from many different health care professionals with conflicting advice. Individual diseases have specific guidelines which can often contradict each other. New medications can be easily accumulated, increasing the risk of side effects, interactions, the need for increasing blood tests and the risk of medical errors.

The aim of these clinics is to help you improve your function. Research shows we can help by

  • Changing the focus of management from disease centred to patient centred.
  • Discussing and setting agreed goals, what do you want to be able to do or improve? What are the key things the GP can do to significantly improve health
  • Focus on improving screening for and managing mood problems as this will improve overall health and management/self-management of other organic illness.

We will be here to listen and support you and your family for whatever health matters arise. Whether you are attending for a new problem or a review, we strive to tailor the latest medical evidence to your care.
